This is accomplished by performing breast augmentation with breast implants to provide the desired volume to the breasts. The lack of breast tissue may hinder an aesthetically pleasing result, so liposuction is performed on areas of the body with excess fat deposits. This fat will be purified and injected into the breast pocket to help smooth the breasts’ appearance and feel.

Endoscopic Breast Augmentation

Dr. Arslanian is one of the ONLY surgeons in Atlanta performing the “No Breast Scar” Augmentation technique. In this technique, Dr. Arslanian can place your breast implants through a small scar hidden in the armpit or axilla. This specialized technique requires special video and endoscopic equipment and requires training that many plastic surgeons do not want to bother with. Keller Funnel

The Keller Funnel is a device designed by surgeons for surgeons. The Keller Funnel is a polymeric film with a hydrophilic coating that allows a surgeon to place a silicone breast implant through a small incision into the breast pocket. This technique is quicker than other forms of augmentation, reduces the trauma to the body during surgery, and can be performed with smaller incisions.

“No Touch” Technique

The “No Touch” Technique is something that separates the average hurried surgeon from the detail-oriented surgeon who has your safety in mind. The Keller funnel is a major component of the no-touch technique. It allows the implant to be placed into the pocket without trauma and without the possibility of contamination from skin, gloves, or instruments. There is literature supporting the role of both antibiotic irrigation and betadine irrigation in preventing implant infections and capsular contracture. Dr. Arslanian uses both throughout the procedure to ensure the implant is completely sterile and has a minimal risk of contamination. 

Studies have shown that even after skin sterilization before surgery, the nipple-areola complex continues to produce bacteria during surgery. The implant could pick up this bacteria. To prevent that, Dr. Arslanian places occlusive “nipple shields” during surgery so there is no chance the nipple complex can contaminate the implant.

An often overlooked component, meticulous technique, and the prevention of ANY bleeding during surgery can impact results. Blood is believed to contain an inflammatory factor that may contribute to the development of Capsular Contracture. In addition, blood is a medium for bacterial infection, making it more likely that a few random bacteria can multiply into a full-blown infection. Dr. Arslanian believes in meticulous dissection to prevent ANY blood loss that can contribute to these problems.

“Scarless” Breast Augmentation

Scarless breast enhancement can be accomplished through a technique known as “TUBA” or transumbilical breast augmentation. In this case, the incision is made within the belly button, allowing the implant to be inserted and tunneled up to the chest region without obvious signs of surgery. Because the TUBA method requires extensive expertise and skill, it is not offered by all plastic surgeons. Dr. Arslanian is one of the few body contouring specialists in the area who has mastered this challenging protocol. With TUBA, saline breast implants are used, as they can be inserted unfilled, limiting the length of the incision.

Fat Transfer as an Alternative to Implants

  • Silicone Gel-filled Implants: Silicone breast implants are silicone shells filled with a plastic gel. They tend to have a more natural feel, are softer to the touch than saline implants, and have a lower chance of rupture.
  • Gummy Bear Implants: A subset of silicone breast implants, gummy bear implants (also called cohesive silicone gel implants) maintain their shape even if the implant shell is broken due to the consistency of the silicone gel, which is thicker than that of traditional silicone gel implants and makes them firmer.
  • Saline-filled Implants: Saline breast implants are filled with a sterile saline solution, similar to saltwater in the human body. They can either be pre-filled or filled once in place, so the sizing can be varied after they are inserted into the breast pocket.

Implant Placement

Implants can be placed in three different locations of your chest: behind the breast tissue, subglandular, or submuscular (beneath the pectoralis muscle). Submuscular placement is often preferred because it can reduce the risk of scar tissue contracture and make mammography easier. A subglandular approach is often the better choice for patients with sagging breasts.

Incision Placement

The placement of your incision is important because it not only allows Dr. Arslanian to perform your breast augmentation most efficiently, but it also determines where and how your scarring will look.

  • Inframammary: An incision made at the bottom of the breast near the breast fold. This is the most commonly used incision because it allows for more accurate implant placement.
  • Areolar or Periareolar: An incision made around the edge of the nipples. This incision serves as the implant’s entry point.
  • Transaxillary: This type of incision is made in the crease of the armpit where the implant is inserted.

Now that you know a little about breast augmentation, the first step to getting surgery is to book a consultation with Dr. Arslanian. In this session, you will learn about the surgery, the specific techniques used, and the types of results previous patients have achieved at our practice. Dr. Arslanian will take detailed measurements of your breasts and body and perform a complete examination. 

Photos will be taken to help Dr. Arslanian point out certain things and explain his approach to breast augmentation. How Do I Prepare for My Breast Augmentation?

Proper preparation plays an important role in ensuring a smooth surgery and an optimal recovery. During your consultation, Dr. Arslanian and his team will provide you with personalized preoperative instructions tailored to your health history and surgical plan.

In general, patients are advised to stop smoking and avoid nicotine products several weeks before and after surgery, as nicotine can impair healing and increase the risk of complications. Pre-operative lab work or medical clearance may be required prior to surgery. You should also plan for time off work, arrange for a responsible adult to drive you home after surgery, and set up a comfortable recovery space at home with essentials such as prescribed medications, supportive bras, and easy-to-prepare meals.

What to Expect on the Day of Breast Augmentation Surgery

The process of getting breast augmentation begins before the day of your surgery. You will be given a set of pre-operative instructions to prep your body for the procedure, which can include fasting, avoiding alcohol, and other medications as specified by our team.

Once you arrive at our clinic in Atlanta, our care team will make you comfortable and prep you for surgery. Dr. Arslanian will review the techniques and incisions you both discussed according to the plan he prepared for you. 

First, you will be administered a type of anesthesia to keep you comfortable and reduce the possibility of feeling any pain.

Once you are comfortably numb, Dr. Arslanian will make the incisions. These will vary from patient to patient, based on the type of breast implant, the specifics of your enhancement, and the existing shape and size of your breasts.

From there, the implant will be placed in a pocket of space opened up by the incision, either under the pectoral muscle or between the pectoral muscle and breast tissue, using a “No Touch” Technique and a Keller Funnel. This helps prevent implant contamination and allows for the smallest possible scar. Once the implants are properly positioned, the incisions are closed with sutures, dressings are applied, and you will be awakened from anesthesia. You will be taken to a recovery area, where Dr. Arslanian and his team will provide all the information you need for your recovery period when you return home. 

We advise breast augmentation patients to arrange for transport to and from our clinic, as this is a major surgery, and you will need to rest.

Combine Breast Augmentation With a Breast Lift

In cases where breast volume loss is accompanied by sagging or drooping (ptosis), a breast augmentation alone may not be sufficient to achieve the desired look. That’s where a breast lift (mastopexy) comes in. By combining the two procedures, Dr. Arslanian can add volume to the breasts with implants while also lifting and reshaping the breast tissue for a perkier, more youthful silhouette.

This combination surgery is ideal for women who have experienced pregnancy, breastfeeding, or significant weight loss. It can also correct asymmetry, including cases where one breast is a different size or shape than the other, and improve nipple position. During your consultation, Dr. Arslanian will assess your breast tissue, skin elasticity, and aesthetic goals to determine whether a combination procedure is right for you.

Long-term issues with breast implants can include capsular contracture and breast implant illness (BII), which involves systemic symptoms.

Breast Augmentation Recovery

As an outpatient procedure, you will go home on the same day as the surgery and rest. Over the first week, you will begin a range-of-motion exercise to keep your arms and shoulders from tightening. Dr. Arslanian will also go over breast implant massage to help prevent early capsular contracture. You will follow up within the first week, at which point dressings and any sutures placed on the skin will be removed. You will be given instructions on how to wear your surgical bra and how to increase activity. The healing process is an important time, and it is essential to follow all of your surgeon’s post-operative care instructions to recover quickly and avoid complications after breast augmentation.

Most people can return to work within 3-7 days, depending on whether they work at a desk or have a more labor-intensive occupation. In two weeks, most patients will be back to normal. You can then start increasing activity, exercising, and lifting heavier weights. We do not allow any heavy lifting, bench press, or push-ups until AT LEAST 6 WEEKS have passed to allow the muscle to fully heal. Patients typically report minimal discomfort after breast augmentation surgery, with significant improvement within a few days. There will be some expected post-operative pain that typically resolves within a few days to a week, for which you will be given prescribed pain medication to manage discomfort.

You will need a supportive, non-underwire bra, such as a surgical or sports bra, to minimize swelling and support healing.

6 Tips for Recovery

As important as Dr. Arslanian’s job is during surgery, your recovery period will decide how your new bust ultimately looks. We will provide you with a detailed list of instructions on how to make the most out of your recovery, not just for your end results but also for your health. Below are some of the instructions you will need to follow:

  1. Avoid heavy exercise: The last thing you want is to hurt yourself or cause a new implant to shift or rupture. While mild exercise is encouraged, wait until the recommended time (at least six weeks) before attempting any strenuous activities.
  2. Use compression bras: Dr. Arslanian will tell you how long to wear yours, but support bras provide additional support and protection.
  3. Get prescriptions filled: If you are given a prescription for pain medication and/or antibiotics, we urge you to have them filled before surgery for a less stressful recovery. 
  4. Meal prep: You want your recovery to go as effortlessly and comfortably as possible. Preparing at least a few days’ worth of food before your operation will give you more time to relax.
  5. Drink plenty of water: Water is life. Stay hydrated.
  6. Maintain a positive attitude: While we are certain you will love your new look, it will take some time to see your final results. Maintaining a positive attitude throughout your recovery is important for your mental health and overall well-being.
