A composite breast augmentation is a relatively new technique that involves placing a breast implant to achieve the desired size, and then utilizing the patients own fat to layer over the implant in order to camouflage it and produce a more subtle, natural look to the breast.

Ideal Candidates

The ideal candidate is one in good health without significant medical problems. They should be have reasonable expectations of outcome and understand the potential risks and complications that may occur. The patient wanting this procedure is looking for a very natural look, but might not have enough fat available to perform a total Breast Augmentation with Fat. If they have a small amount of fat, as most of us do, then that fat can be used to smooth out the edges of the implant and create a more natural look to the breast rather than a straight Implant based Breast Augmentation alone.


During your consultation, you will meet with Dr. Arslanian and his staff and have the opportunity to discuss your individual situation as well as your concerns and aesthetic goals. Dr. Arslanian will will take detailed measurements of your breasts and body and perform a complete examination. Photos will be taken to help Dr. Arslanian point out certain things and explain his approach to breast augmentation with fat transfer. If you are deemed a good candidate, he will discuss the various options for breast augmentation and make sure you are happy with your decision. He will also show you photos of other patients before and afters to help determine what size and look you are trying to achieve. After answering all your questions, you will then sit down with the surgical coordinator to discuss timing, pricing, and any other questions you might have.


Prior to surgery you will be given instructions on the day, time and location of your procedure. You will be given your prescriptions that Dr. Arslanian prescribes, and all of your paperwork including consent forms will be signed. In some cases you may need to obtain medical clearance from your physician or specialist, and possibly meet with the anesthesiologist and undergo additional lab work depending on your medical issues and personal situation.


The procedure will be performed as an outpatient in an accredited surgery center with a board certified anesthesiologist in the safest conditions possible. The surgery will be performed under general anesthesia. Pre-operative markings will typically be made the morning of surgery.

After you are asleep, Dr. Arslanian will start by making his sub centimeter incisions in the areas of planned liposuction. He will then perform SAFE liposuction to harvest the appropriate amount of fat and contour your problem areas. The fat will then be processed and placed into injection syringes. He will then make the necessary incisions to place your breast implants. After placing the implants he will then use pin point incisions to strategically place the fat into your breasts in order to improve the shape and contour of the breast and help to camouflage the implants. The incisions will then be closed and you will be placed into garments for compression. You will be awaken from general anesthesia and taken to the recovery room.


You will go home the same day of surgery and rest. Over the course of the first week you will begin range of motion exercises to keep your arms and shoulders from tightening up. You will wear your garments as instructed, typically for a minimum of 6 weeks to decrease pain, swelling, and bruising. Your breasts will initially be swollen, somewhat firm, and a little uncomfortable. This is normal and they will decrease in size as swelling goes down, and soften as the fat becomes incorporated. You will be given instructions on implant massage and start that soon after surgery. Final results typically take 3-6 months but often up to a year.


Patients can expect about 60% fat retention as some of the fat will not survive the transfer. This means that Dr. Arslanian will typically “over graft” the breasts so that the final result is right in the neighborhood of what we were trying to achieve. The implants will initially sit high and be very “perky” but will start to settle over the next 3-6 weeks. Some patients may take up to 3 months for the implants to settle into their final positions. Final results may take up to 6-12 months for all the swelling and settling to occur.


Composite Breast Augmentation typically costs more than standard breast augmentation due to the extra time and work required for this procedure. It is a very specialized procedure that not all plastic surgeons are trained in performing. The advantage is one of the most natural breast augmentations possible and the size you desire.
